Export destination details

The NZMC number of the new GP must be used to ensure it reaches the correct GP.

Destination HealthLink mailbox must also be accurately entered, in addition to the First and Surname of the requesting GP.

This is critical information, so the patient record is sent to the correct GP who requested it. This information is not validated, so an error message is not displayed if an incorrect EDI is entered.
