If a child has been referred from the Newborn Hearing Screening Programme (UNHSEIP) for a potential hearing loss, their family will already have provided consent to include them in the database. If a child has not been referred from the UNHSEIP, the audiologist will need a signed DND consent form. 

Once consent is obtained audiologists then complete an online notification form which includes information about the child such as their age, the type and severity of the hearing loss and their demographic details. 

Audiologists are strongly encouraged to send the information to the database as soon as possible following diagnosis.  

Reporting

An annual report is published, containing information about diagnoses made during the previous calendar year. 

These reports are publicly available through the New Zealand Audiological Society (NZAS) website

No identifiable data is contained in the annual report.
