What information we collect

When you complete the Survey, Te Whatu Ora collects information about your recent experience with our service (e.g. if the Heart Health Plan is easy to understand) and any feedback you have on how we can improve the service.

You may submit the Survey without providing any personal information to us. Where you voluntarily provide personal information to us, we will manage it in accordance with the Privacy Act 2020 and Health Information Privacy Code 2020.

Demographic Information

When you complete the Survey we link this to your Heart Health Plan via a unique identifier.

We will use your survey responses together with your demographics data including your age, gender, ethnicity and your area of residency, as well as your cardiovascular disease risk assessment (CVDRA) score to analyse your feedback.

Choice

As part of the Heart Health Plan Service testing, we ask that you complete the Survey.

Who do we share information with?

Individual survey responses will only be available to a limited number of Te Whatu Ora staff for the purposes outlined above.

 

Aggregated reports on feedback may be shared with our trusted support providers who help us to provide the Heart Health Plan service, and health providers involved in Heart Health Plan service delivery.

How is your information stored and protected?

Your feedback data will be securely held and managed by Te Whatu Ora in compliance with Te Whatu Ora standards.

How long will my information be kept for?

You feedback information will be kept for no longer than 24 months.

If you have any queries or concerns about how your survey responses are managed, please email ltc@health.govt.nz or talk to your health provider where you received your Heart Health Plan.
