New or renew access holder

You need to update your information every year, by March 31, to make sure you are compliant with national and regional access requirements.

To cover all the facilities in the region, you need to send your updates to 2 email addresses:

Complete and sign the national access agreement, with the names of 2 referees, along with the information in the other sections on this page.

Each year, by March 31, provide renewed copies of all forms for each facility you need access to.

You also need to complete the following if you are registering for the first time.

What you need to provide

You need to provide copies of your:

  • current annual practising certificate
  • indemnity insurance
  • vaccination proof, or confirm your acceptance of the conditions or entry
  • completed children's work safety check.

Children's worker safety check

All access agreements require a current children's worker safety check.

If you are employed by Health New Zealand | Te Whatu Ora, this can be accessed by your HR department.

ICT and security access

The following information is required for computer and security access for each facility.

Wellington Regional Hospital, Kenepuru Community Hospital, Paraparaumu Maternity Unit

The following forms must be completed and signed.

Wellington Regional Hospital parking permits are granted by need. If you would like to request a call back park permit, read the parking policy and complete the request form.

Call back parks are dedicated to staff who are called back while on call.

Hutt Hospital

The following forms must be completed and signed.

To request parking at Hutt Hospital, complete the carpark application form.

To access these, log in to Citrix using your Health New Zealand | Te Whatu Ora username and password. 

You will prompted to set up multifactor authentication the first time you access Citrix outside the hospital.

Homebirth support

We provide homebirth consumables and loan equipment for use in the community by lead maternity carers.

Lead maternity carers who have access agreements and clients planning a homebirth can be supplied with:

  • birth packs
  • loan pools
  • pool liners
  • pool covers.

To order homebirth consumables and loan equipment, fill out the order form and email it to homebirth@ccdhb.or.nz

The team will confirm your order and let you know when you can collect it.

Orders can be collected 8am to 4:30pm, Monday to Friday.

Information about ordering

Orders can be sent at any time, but will only be processed after 32 weeks gestation to make sure there is fair access for everyone.

There is a minimum of 3 days between order and pickup.

The order will be confirmed by email. You cannot pick up your order before receiving the confirmation email.

Equipment must be collected and returned by a lead maternity carer, midwife or medical representative. Loan equipment agreements are with the lead maternity carers.

For midwives supporting high numbers of water births, you can order disposable liners and covers for your own birth pools, if preferred.
