CareConnect

CareConnect is an initiative by Health New Zealand in the northern region (Te Toka Tumai Auckland, Counties Manukau, Waitematā, Te Tai Tokerau) to improve information sharing among the community and hospital healthcare providers to facilitate integrated patient centred care.
