Our public health nursing service works collaboratively with schools, whānau, and healthcare providers to support the health and wellbeing of all students, particularly those who face barriers in accessing traditional healthcare services.

Public Health Nursing Referral Criteria

We ask that schools please verify the following steps have been taken before referring a student to our public health nursing service:

Before Referral

  1. Primary Care Consultation: The whānau has attempted to access their current General Practitioner (GP) or primary care provider for the student's health concerns.
  2. Other Relevant Services: The whānau has explored other relevant health services, such as counselling, specialist care, or community health programs, as applicable.

Referral Process

If the whānau has taken these steps or for some reason is unable to and still requires support, please complete our referral form and provide relevant documentation, including:

  1. Reason for Referral: A clear description of the student's health concerns and reasons for referral.
  2. Previous Attempts: Details of previous attempts to access primary care and other relevant services.

By following this process, we can ensure that our service is used effectively and efficiently, and that students receive the support they need to thrive.
